How do I get an EZproxy HECVAT report?

This article pertains to both hosted EZproxy sites and stand-alone sites that run EZproxy.

The HECVAT for OCLC Hosted EZproxy is a standardized questionnaire designed to evaluate the security and privacy practices of cloud service providers in higher education.  There is no distinct version for self-hosted (stand-alone) sites.  Stand-alone sites have the flexibility to tailor the questionnaire to their needs by adjusting details like Vendor Name, Product Name (switching from Hosted EZproxy to self-hosted EZproxy), Vendor Contact Name, and so on.
